The number of checked boxes in each column determines the total score for each section, e.g., in the section on hand washing and drying, 2 items were marked "Satisfactory" and 1 item was marked "Needs improvement." The total score is 2/3 or 67%. We determine the total score for Infection Prevention by adding all the items marked "Satisfactory" and dividing by the total number of items in the section. 2. If all the conditions in a multiple component question are not observed, the supervisor marks the "Needs improvement" column and makes remarks in the "Observations" column to provide necessary feedback.
